[V8] narrowing down the cause of tranny limp home mode

The other night we drained the tranny fluid and refilled it with 4 quarts per the lists
recommendations. What I now notice is that when the car is cold, it seems to shift at least 1-2-3
without issue. It will also downshift into a gear if you press hard enough on the accelerator
(something it does not do when it goes into limp mode). After you drive it for a few miles, it
goes into limp mode. If you shut the car off and restart, sometimes it will go back to shifting
ok, sometimes it won't (I assume a hard code vs a soft code).
If it is of any importance, when it does shift, it shifts fine - very smooth, very quick. This is
the same with up and downshifts.
Today I tried to pull the main ecu as well as the tranny ecu in a hope to reset any hard codes,
but within a few miles of leaving the office, the limp mode came back. It shifted ok for the few
couple miles, stopped for a red light, and immediately upon accelerating again, the limp mode was
back.
Any further insight into what this might be? Any reason it would shift 1-2-3 but not 4?
