[V8] Failed Emissions

The 91 failed the emissions test this morning.

The HC  and CO were out of spec (According to the state of Colorado).

What diagnosis procedure should I follow to get it back in line?

HC was 6.4 g/ml and 
CO was 179.2 g/ml

A year ago I replaced the O2 sensor.  Could it be a bad ground on the O2?.  Could this be the Head temp sensor?  

Any help is appreciated guys.
