Try this:
take the black covers off your distributor caps. You don't need them, and they actually cause the caps to short out and cause all sorts of strange behavior.
In my case I was getting a code for too rich mixture - along with many of the symptoms you describe.

On the starter - I hope it is just the drive gear.  The only other choice is missing teeth on the ring gear.
