I picked up a 92 Golf diesel dirt cheap a few years back that had the same
thing happen. The original motor had burned out at about 325,000 km
(damned short ratio trasnmissions in those cars) and the owner had a
rebuilt solid lifter from an 85 installed. One of the inserts dropped
into the piston (at high rpm) and battered the hell out of the head, the
piston and the cylinder wall. I still have all three parts floating
around somewhere. The head had been rebuilt by some operation that looks
like they had failed to peen the inserts properly after reinstalling them.

I dropped another solid lifter motor into it after redoing the big-end
bearings to account for the .35" of end play (those thrust washers were
only wafer thing... :) ) and sold the car for quite a profit.
