  The screw is just for convenience.  The lug bolts go through and pull 
everything down tight.  Sounds like a bad wheel bearing.  Jack up one 
side at a time and in about 3rd gear, run it up to 40 or 50 indicated.  
Then put it in neutral and shut off the engine, run out front, and listen. 
Do this for both sides.  If one doesn't seem all that loud, or too loud, 
then you can compare sides.  If they both sound the same then you 
have to determine if it's both or neither!  : )
