You are correct about the cracks between the intake and exhaust valves, this is normal. I would have mine pressure tested though. You asked about the cam bearings, mine never had any and (I have never seen that before on a gasoline head.) I would replace all the valve guides since they are inexpensive ( just a few dollars).
 
As far as milling the head goes, you are correct that you are not to plane the head according to VW, but you can with no problems. The pistons do rise above the bore just a little and there is a special tool to measure this rise. Taking just 3/1000 should be ok, but to make sure you need to measure it.
 
If the piston height above the block is 0.67-0.80mm (0.026-0.031 in) takes a 1 notch head gasket which is 1.4mm (0.055) thick.
 
0.81-0.90mm (0.032-0.035 in) needs a 2 notch gasket which is 1.5mm (0.059 in) thick.
 
0.91-1.02 (0.036-0.040 in) needs a 3 notched gasket which is 1.6 mm(0.063 in) thick. 
 
You might want to ask around to see where diesel mechanics take their diesel heads. That is what I did and most will tell you the same places and most agree where not to take your parts. Ask the VW dealership where they take theirs. I hope this helps.
