  Cracked flange would be my guess. Look real close. I had this same
problem.

  ... Bob  that seems to ring a bell somewhere... I feel I had thi problem
in the past on something... It was a product of corrosion- nip-up ... nip
up seal-up nip-up nip-up  remove inspect  reinstall nip-up swear inspect
and finally realise that the face was bannana shaped from overtightening
and creating a crack that was all but invisible ...until NIPPED UP. Duhhh.
