There should only be *one* TDC mark on the flywheel that shows up once per
engine revolution.
You really have to work at it to get the cam to crank relationship off 180
degrees, unless you've had the head off. Most likely the crank and cam are
okay with regards to one another, and the injection pump is 180 off. There
is a tiny notch on the pump side of the pump sprocket that is used to assist
in getting the pump in time- the notch lines up with what looks like a split
in the pump case at the very top, then you put the pin in. If you can't
find the mark, clean clean clean, then look, if you can't see it clean it
some more. Then if still not, give the pump sprocket 180 degrees turn, and
look for it, clean clean etc... It's gotta be there. Like I mentioned, I
doubt you have the cam to crank relationship off, otherwise the pistons
would make funny noises on the valves that would get quieter as you crank it
over.

Is it possible that the IP, when rebuilt, was
> 180 degrees off?

Nope. Can't.

 Any other suggestions?

Nothing that would be helpful, just tongue in cheek humor stuff.
Have another look at it, seems sometimes if you get some fresh light on it
the next day you catch it right off.
