At the very bottom of the page there is a place for your email address
and a box that says "Unsubscribe or edit options"  Enter your email and
you'll get to the "Unsubscribe or edits options" page.  There you can
send yourself (or anyone) a unsubscribe link OR a password reminder.

I've sent you your unsubscribe link.  You've got to open that email and
finish the job yourself.
