Forgive me if I'm stating the obvious, but perhaps the EGR and vacuum
issues are related? The surging sounds like the EGR opening and closing
quickly as it is commanded open but quickly runs out of vacuum due to the
aforementioned leak.
Doesn't really make sense that the problem would stay gone however, unless
the computer remembers the problem and has stopped using the EGR, or maybe
the vacuum issue is intermittent.
As for finding the leak, maybe you can pressurize the system with air and
use the old soapy-water spray method?
