The countersunk screw is just to hold the rotor in place before
the lugs are back on.  It may be rusted tight, so be sure to apply
some penetrating oil and let it sit a bit.

Once you have the wheel and countersunk screw off, if the rotor doesn't
plop right off into your hand take a small rubber mallet (or if you
live in the NE US, a large hammer!) and tap the rotor head on,
ie, where the lugs go.  It should come right off.
