The throwout bearing and fork sound like a good place to start to me...
 There really is no other explanation, the clutch assembly is pretty basic,
you may also want to flush the hydaulic clutch as well, air in the line can
wreak havoc....  And ck the slave cylinder for full range of play....  Unless
you are doing 6krpm pedal drops, a clutch should get you well over 100k
