Well, the milky white residue IS water, and means you've got a leaking head
gasket, among other things... For the smoking, well, if your car actually ran
after you pulled the oil filler off (it should have stalled instantly) then
you've got a righteous vaccuum problem somewhere! First place I'd check is the
T-hose where it mounts to the top of the valve cover, back of the motor. This
T-hose also goes to the idle stabilizer. These hoses can develope cracks over
time and wreck havoc with idle and other things. There is a hose that runs
under the idle stabilizer, it's about ten inches long, and looks like the
radiator by-pass hose on a Ford Taurus (hint, these are cheaper than an Audi
unit.) Check that, they like to collapse with time. Check any and all vaccuum
hoses in the engine compartment for leaks AND clogs, either will foul things
for you! Best o' luck.
