I guess it depends on the car, too.  In my "other" car - a 83 280ZXT - 
even my size 8.5 shoe fits neatly across the gap between brake & 
throttle in a vertical position.  All I have to do is rock the foot 
left/right to modulate pressure...best pedal setup I've ever driven.  
OTOH, I have driven a couple of GM manuals which had the brake pedal 
about 3" higher than the throttle.  CLEARLY, the folks at GM haven't 
any clue what the phrase "heel & toe" means, nor are they interested 
in learning.  Another reason that I've driven Z-cars since 1974 
(with a deviation from 1977-83) and we've had an Audi for a family 
car since 1987.
