I would suspect one of three things ... but the first thing to do is to 
do a residual pressure test on the fuel system.  You will probably find
the residual pressure to be below spec.  The three things I can think of
that might cause this are: 1) leaking fuel injectors, 2) bad check valve 
in the fuel pump, and 3) fuel pressure accumulator stuck.  I guess some
cars have a metering valve to return fuel to the tank ... that might do 
it as well.  Bentley has a good procedure for nailing this down ...
