Got it in the mid 80's at Auto parts store, under $10.  trimmable
"shroud", base tilts, I glued it to my plywood speaker baffle.  It can
be screwed down, too.

btdt, it was pretty easy, considering how easy anything is when your
lying on your back under the dashboard with no elbow room.  Have you
tried just adding a thick rubber washer or two at the cable stop? 
Voila, more adjustment range!  I was disgusted that I had to do this
with my *new* cable to get full throttle range of motion.
