Hydraulic line failure

Over the last week, I have had the high pressure line from the hydraulic
pump to the steering rack completely fail on me.

First it leaked at the pump end, which I hose clamped and it stopped. It
then started leaking at the rack end, so I clamped that and it stopped. Then
it just split, about 6" back from the rack.

Is this a normal failure mode for a hose like this? It seems strange that it
would fail in all those places like that at the same time. Just wondering if
there could be another problem, like too much pressure or something else.
