Euro Lights From Metrix - What Info Does Metrix Need to ship to U.S.

Hi All:

I'm planning on calling Metrix this week to order euro lights.  I have
two places that are willing to allow me to use their name, etc. to
order from Metrix.

What Information do I need to order?  Name of shop? Tax ID number?
