Transmission "clunk"

Hello There!
I have an A4 Avant 2.8 30V Q with this really annoying problem: Almost every time I shift from one gear to another there comes this "clunk" from the transmission. It is as if there is too much play in the transmission. It is also as if the point of clutch engagement is too high (do y´know what I mean?) so it is hard to engage gears in a smooth way; and this makes the "clunk" problem worse. Also by acceleration the problem appears.
The dealer has replaced the rear differential without results and they seem to have given up. They also say that the clutch is unadjustable.
Can anyone help?
