Saw it once on a car that turned out to have a shimmed wastegate.

It would go to boost once, then when the ECU tried to limit boost
it found that it couldn't and so it shut everything down.  Then it
would hardly tolerate any boost at all until it was switched off and
restarted.

Perhaps the boost --> WGFV line is damaged.
