I had piston slap on my '85 Golf, straight from the factory.
The term refers to "loose" pistons slapping against the
sides of the cylinder bore. It sounds a lot like knocking
or something rattling in the engine. In my case, it was because
the *!&@#^*!&@#^!@ factory put the WRONG PISTONS into an entire
model year's worth of engines. They were 1 size too small.
In this case, I would suspect that either the cylinder walls
have worn (bad rings, over work, bad oil) or the rings have
worn (same suspects).
I think he should have the engine torn down, and have the
rings and cylinders measured. Then make a decision.
