HELP! clutch on floor after 4 days!

Hi all,
I have a weird problem with my 88 80q,
in a week the clutch has gone from perfect
to sitting on the floor, almost can't even
get it in or out of gear!!
What the heck is going on? all i've done recently
is change the tranny oil.
also when I shift from 2nd to 3rd ABOVE 4500 rpm
the gears "grind" for a second before engaging
any help on that?
