Yes, one of the most frequent complaints (and quattro list threads) about '80s
and early '90s USA/Canada Audi models is that the aerodynamic-design DOT
approved headlights are poorly designed (and can be downright dangerous),
especially as the reflectors age/deteriorate and the front glass gets
sandblasted.  If you have a USA or Canada spec '91 200q, then the car came
stock with the DOT lights.
Two good solutions:  
1)  replace the lights with european code lights; or
2) add auxiliary driving lights/low beams.
Some people have found that a modified 9007 (?) bulb in the DOT housings give
a bit better lighting; no experience with that, sorry.

Another thing to consider is that the power for the lights runs through the
wiring harness, through the light switch, and back out to the lights using
small wiring; instead of the normal 13.5 volts produced at the alternator, the
bulbs are likely seeing less than 12 volts.  Adding a relayed wiring harness
will take the current load off the wiring harness and light switch, and
provide the lights with the proper 13.5 volts (which may be worth a 25%
improvement in light output, alone!).  This is strongly recommended to anyone
running higher wattage bulbs than stock, and a good idea even with stock
lights/bulbs.
