Its a vacuum leak in the sysyem - usually caused by the hose coming off the
vacuum motor for the recirc door, or the motor itself going south. A quick
check can be accomplished by peeking in the top door in the engine bay under
the plastic cover on the passenger side.
In my experience, this area is where many problems occur due to the exposure
to the elements. The door seal goes south, the vacuum motor rubber ages and
tears, the hose hardens and falls off. Another problem that occasionally
crops up is napkins getting sucked up from the passenger floor area and
blocking off air flow against the heat exchanger (often leaves will migrate
in and do the same)..

Return spring failure causes fogging.
