Paul:

What caused the failure of the outer CV - was it not properly attached to
the hub and it "pulled" out?
BTDT with my CQ - found out the hard way after doing some suspension work
and letting the hub/cv/a arm "hang" - the circlip pulled out on the CV
joint, went to move the car and ...nothing....looked underneath and saw
that the CV balls were out of their carrier.
