ISV is a good place to start, but this could be a vac. leak of some
sort...in hoses, intake, even worn bushes in the throttle body. Maybe it's
an issue w/ the over-run valve on manual tranny's. It could be turning on
due to bad wiring or just a sticky valve. Somehow it seems air is getting
into the engine w/o the control of the throttle valve. Intake hoses and
such need very careful examination, b/c the leaks/cracks usually occur
where you can't see them and many times under the hose clamps. I think a
very close inspection of the whole engine should turn up something, even if
it's not an intermittent leak.
My car had intermittent idle issues due to a cracked intake hose on the
turbo that would seal and loosen as I was driving...good luck...you'll need it.
