The most effective method is with the use of a cleaning tool, it has a
nozzle that goes in through the spark plug hole and shoots a stream of an
abrasive cleaning medium (ground up walnut shell usually) directly into
the cylinders to clean the pistons and valves. This tool was much in use
a few years ago primarily on certain BMW's that had a problem with
buildup which caused hard starting among other problems. You'll have to
find a shop in your area that has the tool. I don't know where you're
located, but I know of several shops in the Boston area that have the
tool. The medium used doesn't do any damage to exhaust or internal
components.
As another lister mentioned, you should first try the available cleaners,
there is one brand (the name escapes me now) that is a three part
treatment, it comes with a charged cannister of cleaner that you attach
to the pressure side of the fuel system and run the engine. Then you put
the second can of cleaner in the gas tank and one in the oil. It's quite
a sight, I saw a Mercyless Benz being purged and the exhaust billowed
like a crop duster while being cleaned.
