He mounted it in the intake manifold. Near the top I think where there is a
boss that looks like the factory intended for a sensor to go there (and
probably did put a sensor there on some cars):

sure.

Closest to the intake manifold as better as it gives the most acurrate
reading of the temperature of the air just before it enters the engine (which
is what the ECU cares about).

he bought his from Fodge initially but I think I pointed him to the cheapest
source that I found which was Force Fuel Injection. Pretty nice people to
deal with. No hassles. If you need a fuel rail made up I have all of the
appropriate size drill bits and taps and some spare rail stock. Email me
privately if interested.

yes. That's what I'm going to have to do on my 4ktq with 5kt ignition
system. I may eventually convert to the 90q ignition setup as it has some
advantages (one being no need for a cam trigger). The one I made up is
viewable here:

If I were you I'd go with the 90q ignition system if you are going to convert
from stock. The ignition system on the 90 is discrete from the fuel system
which makes for a nice clean installation. And it is knock sensing.

Don't think so. I believe you can use the 200/5kt flywheel with the 4kq
ignition system.
