Injector Tip Fell Off - HELP!

The later comment about the spark plug thread surprise suggests you haven't
had the car long?

Yeah, it's gone.  I would suggest it got caught between the piston and the
plug, popping the latter out, and then got destroyed (they seem to 'flake')
and dropped down between the piston and the walls, gouging cheerfully away.
The black is burnt oil rather than richness.  A leakdown test would be a
good idea, but I think it's head off at minimum.
