HI Nate,
My guess is that the ISV is gummed up and not working correctly. The hole in
the intake hose was getting the engine the extra air required that's
supposed to be provided through the ISV when the throttle is closed. Now
that the hole is gone, the engine can't get the needed air. That would
explain the need to step on the gas to start the car now, too. It would also
explain the unstable idle.
So, clean out your idle stabilizer valve with carb or throttle body cleaner
and try again.
