I've seen this when people have allowed the piston rod to rotate a little
when tightening the top nut - doesn't need to move much to loosen the
slotted nut and then each slight movement loosens it more. When they finish
they have a tight top nut but the slotted nut has backed off by a turn or
so.

I am at the end of my rope with this one. The slotted nut on my left front
strut insert will not stay tight no matter what. I discovered a clunk from
the spring about a week after I installed new springs/inserts. Pulled the
top nut and cover and sure enough the slotted nut had become VERY loose. I
dropped the assembly out from the fender, compressed the spring, retightened
the nut to spec with the tool and reinstalled using the proper tools to keep
the nut from loosening itself. The next day, the clunk is back. Removed
the top nut/cover and the slotted nut had come loose once more. I dropped
the assembly yet again. I inspected the bearing to make sure it spins
freely and it does. It was installed new with the springs/struts. The
thing won't stay put for more than a day. I'm guessing that while I am
making turns with the car, the nut is coming loose for some reason. Anyone
else go through this? It is only occuring on the driver side. Did I get a
faulty bearing? Any help would be appreciated.
