bad noise from turbo was loose EM

Hello,
I managed to rip apart the entire intake tract on my Quantum Syncro turbo today before I found out what the nasty sound was. The #1 cylinder exhaust manifold nuts were very loose. I bet the other cylinders might be the same. My question is why? I torqued them when I put it all together. Could the gaskets be falling apart? Also, on my #3 cylinder, the lower stud broke off during the manifold swap. I just left it since I didn't wanna take off the head. Could this contribute to loose nuts?
