I use a small (12" to 14") pipe wrench through the spring coils on the top
cap.  I use a 36" length of pipe on the handle of the pipe wrench as a
cheater bar to break things loose.

Oh, and on the nut at the top of the shaft, I use the same pipe wrench on
the outside of a cheap 6-point socket for the nut with an Allen wrench
through the middle of the socket to hold the shaft. Sometimes I have to use
short lengths (12" to 18") of pipe as cheater bars on the Allen wrench and
the pipe wrench.  If the shaft hole for the Allen wrench strips out, take
some rubber, like from an old bicycle inner tube, and clamp the shiny shaft
down by the spring with a set of big Vise Grips set pretty tight. I always
do my rears off the car using rubber and Vise Grips to hold the shiny
shaft.  If I need more clamping force I add another pair of big Vise Grips
and rubber or a big pair of Channel Locks (slip-jaw pliers) with rubber.
Who cares if you scratch the shiny shaft on the old shocks? The old ones
are junk anyway.

BTW, these methods have never failed me the six or seven times I have done
this.
