After looking at the pics, I'm thinkin I'll round up some bits and fire up the tig....
After checking with the stealership, turns out that the fork is NLA. Based on that seems like it's time for a solution that doesn't require and obsolete part...

Thanks a bunch Cody!

Yes and no. I've got a 3B flywheel and pressure plate in my 'fast'
5ktq and it works fine. You will need to modify the slave cylinder rod to get a
little more throw distance or the clutch will not fully dissengage, but beyond
that it's no problem. This is because the release bearing is about 1/4"
thicker on the later setup, so you just need to make up that distance. You COULD
(and I'd recommend do so) switch to the later style throwout fork and
bearing, but it's not required.

On mine I made an adjustable length slave cylinder rod:

If you need I could make you one like it, or if you have the tools feel free to
copy the idea ;-). It's a bolt welded to one end of the rod and a stack of
nuts TIG welded together and to the other end of the rod, easy peasy. Oh,
there's a locking nut in there too. It was nice to be able to fine tune the
length to get it just right without having to make 5 different length rods.
