The first step is to do a fluid flush.  That's basically a brake bleed but
the bleed port is on the clutch slave cylinder instead.  The clutch master
probably is fed by a hose that comes off the brake master cylinder.  Getting
to the clutch slave cylinder is usually a bit of a pain and the bleed screw
could very well be corroded/frozen, so be prepared for this to take some
time, sweat, and patience.

Ultimately, it may be necessary to replace the master and/or slave
cylinders, but doing the flush and checking the condition of the fluid that
comes out and whether or not flushing seems to improve your shifting action
is the first step in determining whether or not you need to replace parts.
