The replies so far outline the process fairly well so I've only got a couple notes.
In a system designed for R12 you will find that R134A does not cool quite as well. Changing to a R134A spec condenser can solve that, but isn't required if you find that it cools sufficiently as is.
There is another option called EnviroSafe. I've always wanted to use it, but I ended up removing my A/C instead about 7 years ago and my other 5000 has a dead compressor so i haven't had the chance yet. I've heard good things.
