On my car the odometer works all the time so I know the problem is in the gauge.
FWIW my spare totally stopped working today. Heavy sigh...............

Pull the cable out of the trans and verify the gear in the trans and the one on the cable adapter piece are fine. May be worn to the point of occasionally slipping/jumping.

Mine was the same way. Couldn't be replicated at the shop. I told them I would like it looked at/rebuilt anyway, and that is when he basically blew me off with a really high $$ amount. More than double the cost of a few years ago.

One idiosyncrasy on mine was that it was a bit temperature dependant. It worked better in the cold. Maybe they should warm yours up? Sounds crazy but this whole thing is a little bit of that already.
I was certain mine was the speedo and not the cable or sender at the tranny since the odometer always worked.
I put my spare in the car which I thought had a bad ODO gear, and I like to use the trip to calculate MPG for each tankful. Helps me keep tabs on the health of the turbo motor (and my leadfootedness). Strangely the speedo needle and ODO of the spare are working perfectly. Not that I'm complaining.

As I said earlier, I'm having this problem with my 4kq. Palo Alto
Speedometer called me today saying they can't replicate it. Since the
odometers work and I had the speedo problem with both the previous
cable and the brand-new replacement cable, I'm skeptical that it's
the cable. I've had good service from my garage and would assume that
they used a good cable, whether OEM or not. I just emailed them
saying that based on my experience, they should try letting the
speedo sit for 30 minutes before testing. Any other thoughts?
