Glaze is the answer and it usually comes from over heating the pads and
glazing their surface making the surface so hard that it vibrates against
the rotor when applied.  This can also happen when the brakes are not used
and just tickled.  After a time the worn material that is not really removed
by the rotor, becomes embedded on the pads.

Solution, go out and make ten stops in sets of three from 50 mph plus with a
stop at #4 and # 8 which is slow and steady.  Then drive for a couple of
miles to let the brakes cool.  Do not stop and leave your foot on the pedal
at any time in this process.

This will bed the pads, and rotors and cure the problem.  Also try not to do
the "Merican" squat every time you stop at lights - sitting at lights with
your foot on the brake when you have used them heavily.  This will promote
the deposit of material onto the pads.
