I agree with Chris. Having participated in an Aerospace Failure
investigation myself there are certain conditions in which things can't be
expected to be 100%, especially when you are pushing the envelope. Hell,
you could make the same argument about commercial aircraft...."Gosh, the
rear stabilizer hangs and what do they expect you to do, just hang on and
die"...precisely.
Although there is a difference, which is that it is probably plausible to
come up with a system for an airliner, but not feasible for the shuttle.
The airliner example is just a cost trade. So if people want to pick on
something, that would be the place to start. The fact is that approximately
1% of the people launched into space have died. I think a 99% success rate
on something like that is pretty damn good. Everybody who flies these
things knows that and signs up for the risk. I am much more worried about
not being able to live life to the fullest because of garbage rules and
restrictions than "living life to the safest". Do you think Lewis & Clark
asked for a guaranteed safe passage through the wilderness before they would
sign up to explore the whole western half of the country? I am going to get
carried away here....probably already have :) So I will stop here.

Unfortunately not everything in life is survivable.
When the astronauts were going mach 18 and the explosion took place,
there was no airbag, seatbelt etc. yet devised that could have saved
them. When they take off it is not much more than a controlled
explosion, when they return the temps are unbelievably high,
unfortunately when a problem occurs during this points in time there is
only so much that can be done.
With an explosion of that magnitude I feel safe in saying that we can
take solace in the fact that it was a quick end and they didn't suffer.
I may be wrong, but I hope that is the case.
