David, Sorry to hear of your incident, but very glad that you and your
family are well.
Another thought into the fray...
In a previous life, one of my customers was the Battelle Memeorial Institute
www.battelle.org. They do engineering and design work for every imaginable
industry. (Food, Military, Automotive, etc.) Anywho, one of the projects
they worked on was establishing the cause of the failure of the cabin door
that blew off of the 747 departing Hawaii in February of 1989. They used
electron microscope technology to determine how the crystalline structure of
the metal had failed i.e. from what direction the forces that caused the
failure had come from, whether it was a single blow or if long-term stress
had caused the failure, and even the frequency and amplitude of the forces
causing the failure. (all this being done, of course, by scientists with
enough letters after their names to win a scrabble tournament)
I wouldn't know where to start with this, that's probably an attorney's job.
I'm just saying that the technology exists to find the root cause of the
failure. Who pays for it? I dunno. I would think that the manufacturer of
the component would want to know if it was their problem, as the next victim
of this failure may not be so fortunate as to escape unscathed.

Yeah,
I'm kind of sorry I mentioned it on the list but I got a bit carried away
with the topic of conversation at the moment. I liked Dave and Pete a lot
and It's quite possible it was no fault of there's at all. I still haven't
decided what to do.
