It was a new injector; put in when the car was rebuilt in 1998.

What happened; while my parents were driving through the Home Depot parking
lot in Kelowna BC, they heard a very large bang. The sound was loud enough
that they thought that they were rear-ended. About a minute later, my
mother said "is that smoke coming from the hood (front)?", my father's
response "nope, that's fire!". The fire was contained to the engine bay, as
the fire department arrived pretty quickly. The car was still written off
though.
During the investigation, the mechanic found that an injector was flawed and
that there was a weak spot (in the housing I assume) that blew out. When
the casing failed (the loud bang), the result was fuel started to spray all
over the place. They assumed that a spark ignited the fuel and the rest is
history. The spark could have come from any number of sources under the
hood. The injector (& Car) worked just fine from the time when the car was
rebuilt in 98' until the fire four years later.
Cheers
RS
