i had this problem with my CD changer - repeatedly.

take the CD changer out and open it up.  there are some rollers covered
with rubber of some kind.  these rollers grab the CD and slide it in/out
of the cartridge.  over time, these rollers pick up dirt and don't grab
the CD as well.  after i cleaned them, the CD changer worked fine and
has been working for the past couple of years.
