I finally got the fix implemented this morning. It turned out to be a
broken wire in the connector boot of the speed sensor. specifically,
the power to the sensor. the broken wire was pinched - no doubt legacy
from the fine folks who replaced my timing belt and subsequently
bought me a new head. (I bought the car new and no one else would've
had reason to unplug the connector) This isn't the first problem I've
discovered - they also bunged up the hall sensor on rebuild.

what are the odds of it happening in the one sensor that's only used
during starting? (which explains why it ran perfectly if you could get
it started)

a little solder, a little tape and it's running fine again!
