FWIW Department,
I had all my serpentine goodies replaced at the dealer last month while
getting the t-belt replaced.
They replaced the tensioner pulley but it failed in less than 150 miles
taking out the radiator with it. Dealer covered all the repair under "parts
warranty" however I had to cough up for a new tensioner bracket which they
said was the ultimate culprit.

Many thanks Fred, JR, Bob
Unfortunately, as you suspected, my problem lies in the tensioning bit.
I'll try to dismantle it so I can photograph what went wrong, but
basically, it's got far too much mechanical slop. This allows the idler
wheel to move up and away from the belt (in the direction of the
radiator), and because of the angle, one edge digs into the belt. The
other edge (radiator side) is actually in free space.
I ordered another tensioner from the dealers - GBP171.16 (I think that's
around USD325). I should get it tonight.
