My money is on a bad sensor. My guess is that all is fine when cold, 
but as the engine comes up to temperature, one of the sensors dies
(or fails to take over from a cold start), reverting to a limp-home 
value of some kind.

What sensors might do it?  The O2 sensor needs to warm up to work.  
Does the S4 have the old MTFS, which frequently fails on the 200s?
