High NOx is normally a result of a bad EGR system, lean running and/or high 
combustion chamber temps. Dirty injectors and/or air leaks are likely 
suspects. At 200k, you may have quite a bit of carbon buildup, too. That can 
be fixed without disassembling the motor by using a chemical flush. A flush 
took care of a pinging problem I had years ago on a hot-rodded VW GTI. Also, 
lots of people swear by a bottle of BG 44K for a good fuel system 
cleaning -- no personal experience here.
