I don't think there is a most common cause.  I've had 10 or so leaks and 
every one of them has been in a different place.  You've gotten a pretty 
good list of places so far.  I'll add to this that you can reach a finger 
down under where a few of the hoses connect to the radiator to check if they 
are wet, and if you find yourself replacing either the plastic fitting on 
the back of the block or the tee in the hose a few inches away, replace both 
of them.  Every time I fix a leak the system is tight for 6 to 18 months 
then I get a new one.  Sometimes it takes months before I find it.  I 
discovered a few drops of coolant on the garage floor a coupleof weeks ago 
and haven't yet a clue where it came from.
