Me thinks (and a dealer guy on one of the other forums tells me that Toyota
apparently has been installing a software only throttle/brake overlap
cut-off, and it's operation is wonky) that a much more HARD overlap control
needs to be installed on Toyota's to relieve any doubt.  As an example, wire
the power to the throttle steppers through a N.O, good old fashion relay,
with is held to power through the brake light switch (IF they even use them
anymore, knowing "modern" technologies, I wonder if even THAT goes through
the ECU, if not, add one). If the brake light is triggered, and the throttle
is being activated, then power to the Throttle stepper motor is killed, and
engine power is killed (down to idle). Problem solved, easily traced. No
software shennanigans.
