I had it saved as one of Fred M's (and posted it up to 12v.org/urs as such) but Fred says it wasn't him.
(He usually signs and dates his pdfs).  The one in question doesn't have anything re: author.

Now that I look at it, it might be Steve M (boxflares). Can you confirm/deny Steve?

I want to give credit where credit is due and Fred doesn't want it.
