I have had that problem myself.
Things to check (you already have the top 3)
4). Is the starter a) the large OE type, or b) the smaller aftermarket
(Kragen, Autozone etc.) or c) later 5k, 200 type?
5) Is the OE heat shield in place?
6) The new ignition switches may still not deliver enough juice when hot. I
use a push button starter switch that bypasses the ignition lock switch.
I have had success with the OE smaller {not aftermarket} (in my case a '91
20V) unit as it allows more air flow to cool, heat shield in place, and a push
button switch.
